He argued that this pattern deserves closer scrutiny.

A spokesperson said the department fulfilled 160 information requests and that 73 staff members collectively devoted thousands of hours to the audit process. The agency concluded by stating it will take constructive elements from the report and continue improving its operations moving forward.
